Team Leader – Medical Degree/BSc in Nursing

Purpose Statement:
Care Bridge Home is a professional organization offering a comprehensive range of medical and non-medical support services for seniors and individuals with various medical conditions, both in their homes and within residential care settings. We are currently seeking a candidate with a BSc in Nursing or a medical degree, along with a minimum of one year of experience in Clinical Care, Care Coordination, and training.
The Team Leader will be responsible for coordinating and implementing comprehensive healthcare services.
This role requires advanced clinical experience in comprehensive case management, patient service delivery, training, administration, medical reporting, and documentation.
The ideal candidate must demonstrate excellence in communication and report writing in English.
Only those with a medical degree or a registered BSc in Nursing, and at least one year of relevant experience, will be considered for this position.
Role Description:
- Develop and implement comprehensive assessments, care planning, and care coordination in collaboration with a multidisciplinary team, clients, carers, and other stakeholders.
- Coordinate and manage optimal clinical care and treatment for seniors, ensuring the needs of the client, their family, and caregivers are met in both residential and in-home care settings.
- Manage and provide interventions and treatments, while monitoring responses to the treatment and care plan.
- Liaise with the treatment team and family members of the patients to ensure continuity of care.
- Lead and empower reporting staff through education, support, and guidance to elevate the level of service.
- Provide induction for new staff and participate in ongoing staff training.
- Liaise with both internal and external stakeholders (including residents, families, contractors, and visitors) in a compassionate and helpful manner that reflects the mission and values of Care Bridge Home.
- Demonstrate a strong understanding and application of Workplace Health and Safety principles in the provision of nursing and care services.
- Develop program reports and training reports, ensuring timely submission.
- Maintain a high level of administrative efficiency and accountability.
- Ensure that the organizational code of conduct and values are followed, implemented, and effectively overseen.
- Perform additional duties as required to fulfill the responsibilities of the position.
- Candidates must hold a registered BSc in Nursing or a relevant medical degree, along with a minimum of one year of experience in nursing services, clinical service, training, and care coordination.
- Possess a professional and caring disposition, with excellent written and verbal communication skills, and proficiency in both English and Malayalam.
- Demonstrate strong organizational skills to prioritize, organize, and execute care plans in a timely manner.
- Ability to interact professionally with healthcare professionals, consumers, families, caregivers, and others.
- Show empathy and a compassionate approach with a broad range of clients.
- Have a genuine interest in working with the elderly and a commitment to holistic care and well-being.
- Ability to work independently as well as collaboratively within a team.
- Willingness to work in residential care settings on a shift/roster basis, including weekends.
- Hold a valid driver's license with a clean driving record.
- Proficient in Microsoft Office systems.
- Three-year contracts with six-month review, with the possibility of extension based on performance.

- Thiruvalla, Kerala: Must be available locally ( thiruvalla) and willing to travel
