Clinical Pharmacist - Care Connect

Job Title: Clinical Pharmacist
Location: Remote (occasional on-site meetings RSA Building 7th Floor, Dundrum, Dublin 16)
Hours: 16 per week
Contact: Perm Part-time
Work pattern: Monday – Friday (hours and days flexible)
Salary: Competitive (based on experience)
Reports To: Medical Director / Managing Director, Care-Connect
Team Structure: Collaborates with Clinical Nurse Lead, Clinical Governance Committee, and Multidisciplinary Clinical Team

Our team of experienced healthcare professionals, led by a Clinical Nurse Lead and GP, provides comprehensive and personalised care to patients. As we continue to expand, we are seeking a passionate and skilled Clinical Pharmacist to join our dynamic team, playing a pivotal role in enhancing patient outcomes and optimizing clinical pathways.


Purpose of the Role:

The Clinical Pharmacist at Care-Connect is a vital member of our multidisciplinary clinical team, specializing in the dispensing of medications, providing essential pharmacological insights, and collaborating closely with healthcare professionals to ensure optimal patient care. This role is key to improving medication management, identifying clinical risks, and supporting the overall enhancement of our clinical services.


Key Responsibilities:

  • Comprehensive Pharmacy Services: Deliver a high-quality, comprehensive clinical pharmacy service to all Care-Connect patients, ensuring excellence in medication management and patient care.
  • Medication Onboarding & Reviews: Lead all aspects of medication onboarding, annual reviews, reconciliation, and pharmacological assessments. Engage with patients, GPs, community pharmacists, and hospital pharmacies to consolidate and review comprehensive medication lists, optimize treatments, and recommend suitable alternatives when necessary.
  • Unscheduled Medication Reviews: Facilitate additional medication reviews upon request from the clinical team or in response to patient concerns, ensuring timely support within scheduled working hours.
  • Collaborative Care: Work closely with the nursing team to monitor patient adherence to prescribed medication regimens and identify potential side effects or adverse interactions.
  • Education & Training: Provide ongoing training and education to the Care-Connect team, participating in team activities, training sessions, and educational opportunities relevant to the pharmacist role.
  • Clinical Governance: Serve as a member of the Care-Connect Clinical Governance Committee, offering expert advice and attending scheduled meetings to address patient medication management issues.
  • Pathway Development: Collaborate with the Head of Pathway Development to support the planning and development of new clinical services and programs, contributing pharmacological expertise to enhance patient care.
  • Service Expansion: Play a key role in strategically developing and expanding the pharmacy services at Care-Connect, responding to the growing needs of the business.


Qualifications & Experience:

  • Educational Background: Master’s degree in pharmacy accredited by the Pharmaceutical Society of Ireland (PSI) with current registration with the PSI.
  • Experience: Minimum of 3 years of experience as a clinical pharmacist in a primary care or hospital setting (Ireland), preferably within a multidisciplinary team.
  • Remote Work Experience: Prior experience in a remote role is an advantage.

  • Analytical Abilities: Strong analytical and problem-solving skills, with proficiency in working with healthcare data to derive meaningful insights.
  • Team Collaboration: Proven ability to work collaboratively within cross-functional teams, with a positive attitude and commitment to teamwork.
