Project Worker, Youth Case Management, Dublin

Contract: Full-time, Permanent Contract
(Starting Salary €36,687 - €39,856)
Provision of Intensive Case Management to young people (18-26) as part of a Youth Case Management Team working across a range of Services internally and externally. The Case Managers works continuously with the same young people when they enter the service until they leave.
What you will do:
To work within the framework of the overall objectives of Focus Ireland.
To work within the overall policies and procedures of Focus Ireland, inclusive of Health and Safety Policy and ensure their implementation
To establish trusting relationships especially with young people who are living on the streets or in temporary accommodation and who are unattached or out-of-home and engage them in work that enables them to move to a more stable lifestyle.
To carry a designated case load.
To undertake assessments and support planning with young people as the lead agent using a Case Management system and protocols and other tools as appropriate to assist the young person to successfully progress toward independent living as appropriate.
To complete appropriate referrals to other agencies and services as required.
To develop and maintain professional links with other agencies and services that may offer services to young people. Assisting in the coordination of services for the young people.
To accompany and provide support to young people on appointments to other services i.e., doctors, counsellors, court etc. and to provide support to young people whilst in custody
To be responsible for keeping up to date casework records, daily logs and written assessments. Inputting information on the Focus Ireland Database.
To attend Team, Section, Agency and external agency’s meetings and training as required.
To participate in supervision with your line manager and attend Reflective Practice.
In consultation, to undertake any other responsibilities and duties as may be reasonably assigned by the Project Leader, Manager or Director of Services.
What you will need:
A Third Level qualification at degree level in social care i.e., youth work, social studies, addiction, psychology etc.
1 year’s direct experience in the social care field working with disadvantaged young people.
A strong knowledge of issues affecting young people out-of-home and their impact on young people and their physical, emotional, and psychological development.
Relevant experience of dealing with high risk and challenging behaviour.
Relevant experience of working in a team which shares a common goal.
Ability to cope with and manage lone working including out of hours’ work
A working knowledge of services in the homeless sector. (Desirable)
An awareness of child protection issues and ways of responding appropriately to any concerns. (Desirable)
An awareness of a trauma informed approach to working with vulnerable populations. (Desirable)
