MDT Liaison- Discharge Co-ordinator

An exciting opportunity has arisen within Medicine for the Elderly and we require a motivated and experienced HCSW with a genuine interest in caring for the older person to join our team.
Discharges in MOE can be complex and we require an individual that can plan and coordinate these discharges in a fast paced environment.
Previous band 2 hospital experience essential.
SVQ level 3 is desirable but if you don’t possess this you must be prepared to work towards this. The extended clinical skills required for this post are; venepuncture, cannulation, performing ECG’s and catheterisation. Computer skills are also required.
This post is on a job share basis.

Please note the salary for this post is pro rata to part time hours.
A requirement of this post is to become a member of the Protection of Vulnerable Groups (PVG) Scheme Prior to appointment.
NHS Scotland is reducing their full-time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
