POSITION SCOPE:
The purpose of the Healthy Babies/Healthy Children Worker is to establish relationships with families of Atikameksheng Anishnawbek and provide them with wholistic programming, health, social, and education services support, assessing their needs and eligibility for various types of internal and external agency supports. The Healthy Babies Healthy Children Worker will assist and guide families through processes that help strengthen their family unit.


REQUIRED QUALIFICATIONS:


Mandatory Requirements

• College Diploma in Social/Health Sciences
• One year work experience in service development and coordination
• CPIC (Vulnerable Sector) – must be provided annually and at time of offer

Additional Requirements

• Knowledgeable and understanding of provincial and federal Privacy Laws and Acts
• Knowledge of Electronic Medical Record technology and charting
• Knowledge and experience in case management
• Sign language skills are an asset

KEY RESPONSIBILITIES:


Family Supports

• Complete home visiting program for clients and families
• Provide support and assistance to families through the case management model
• Complete intake and assessments to identify/establish family goals, needs and supports
• Complete referrals as necessary
• Complete clinical administration ie., EMR-Electronic Medical Records, charting, minutes, scheduling appointments
• Participate in case consultations/management meetings with service providers to identify and address issues.
• Address pre-natal care as outlined in the HBHC/Canada Prenatal Nutrition Program work plans
• Monitors progress of growth development with children from ages 0-6 yrs of age
• Work with service providers for opening the Early Years Centre for family scheduled visitation rights

Promotion

• Provides promotional material about Healthy Babies Healthy Children programs/services
• Provides information to expectant and new parents

Programming

• Design, develop and implement a structured playgroup that provides interaction between parent and child
• Design, develop and implement family programs that help them grow healthy mentally, spiritually, physically and emotionally.
• Utilize the Medicine Wheel teachings in all programming.

Administrative

• Preparation and monitoring of annual budgets
• Participate in organizational and Team Staff meetings
• Prepare monthly calendars and participate in program planning meetings
• Record participation statistics and prepare monthly reports
• Enters program and services data onto Electronic Medical Records (EMR) system as and when required
• Contribute to report submissions and meet funding requirements.
