Requirements/Qualifications

A regulated health professional Occupational Therapist (OT) with a minimum 2 years experience
Registered and in good standing with the College of Occupational Therapists of Ontario
Recent experience with progressive responsibilities in managing patients in acute/rehab or community-based rehabilitation programs is preferred
Solid knowledge of principles, practices, and methods of occupational therapy and service delivery to clients.
Knowledge of relevant legislation and community resources as well as an understanding of the discharge planning process required.
Valid drivers license and willingness to travel in Kitchener/Waterloo/Cambridge/Brantford area
Strong time management skills, confidentiality and professionalism
Excellent communicator, able to foster relationships with Home Care Staff and clients
Ability to work independently and work effectively within a team environment
Interest in continuous learning – high energy and focused
Police Criminal Record Check with Vulnerable Sector Screening.

Responsibilities

Conducting assessments and implementations of OT interventions, such as ADLs, IADLs strategies, equipment recommendations, home modifications, etc., in a community setting
Responsible for developing and implementing comprehensive treatment programs
Support a holistic and restorative approach to care ( enable opportunities to restore, create and sustain health & well-being via interventions, health coaching and self-management support)
Collaborates with other members of the health team and referral sources as needed
Evaluate the effectiveness of treatment plans and modify them accordingly
Participate in proactive Health & Safety activities while performing all duties. Notify immediate Supervisor of any Health Concerns.
Complete all other tasks as required.
