Montefiore Mount Vernon Hospital, a community based hospital which is part of the Montefiore Einstein System, is seeking an Stroke Coordinator to join our team.
POSITION SUMMARY:
The Stroke Coordinator is responsible for the coordination of systems and services required to maintain an organized interdisciplinary, patient centered TJC/(NYSDOH) designated Primary Stroke Center. The Stroke Coordinator is responsible for supporting the delivery of evidenced-based acute stroke assessment and management, providing expert nursing consultation and practice oversight, participate in and/or develop performance improvement initiatives, participates in stroke center research activities, and develop and deliver acute stroke continuing education programs.
He/She is responsible for ensuring the stroke program's compliance with Joint Commission, New York State Department of Health, and American Stroke Association standards.
Job Duties include:

• Fosters quality patient care and outcomes on Stroke patients (except ICU's).
• Fosters excellence in nursing practice.
• Participates in Quality Assurance/Performance improvement initiatives.
• Organizes and participates in community stroke education.
• Provides coverage for Montefiore New Rochelle Hospital

Required Background and Experience:

• BSN in Nursing. Masters Degree preferred
• Current NYS RN Licensure
• Certification in performance of the National Institute of Health Stroke Scale (NIHSS)
• 5 years of clinical experience
