The Care Partner - Qatar is responsible for managing the rostering of support workers and ensuring the delivery of quality home care services. This role involves working closely with clients, families, and care teams to ensure seamless service delivery and high client satisfaction.
Key Responsibilities
- Rostering Management: Develop and maintain an effective rostering system to optimise support worker allocations and ensure uninterrupted care delivery.
- Client Assessment and Care Planning: Collaborate with consumers, families, and healthcare professionals to design, update, and assess care plans.
- Consumer and Family Support: Serve as a primary contact for clients and their families, offering guidance, support, and advocacy for care needs.
- Collaboration and Communication: Ensure smooth communication between support workers, clients, families, and the care management team to meet individual care requirements.
- Compliance, Documentation and Reporting: Maintain accurate records, ensure adherence to healthcare regulations, and contribute to quality improvement initiatives.
- Continuous Improvement: Identify and implement opportunities to improve rostering processes, service delivery, and client satisfaction.
- Tertiary qualification in Health or a related field, with relevant experience in care management.
- Experience in home care or healthcare settings, particularly in rostering or scheduling.
- Excellent interpersonal and communication skills, with the ability to work effectively with diverse populations.
- A current National Police Records check (or willingness to apply).

- A strong sense of integrity, trust, and confidentiality when handling sensitive information.
- Ability to work both independently and collaboratively within a team.
- Excellent time management skills, with a proactive approach to problem-solving and service improvement.
