Key accountabilities
- Review medication orders for completeness and appropriateness
- Process drug orders and dispense medications to patients
- Counsel patients (inpatients / outpatients) on their medications and provide specialised counselling if needed
- Perform medication reconciliation
- Provide rug information to doctors, nurses and patients
- Assist in the preparation of ad-hoc items e.g. extemporaneous preparations, reconstituted antibiotic suspensions
- Other duties as assigned
- Bachelor of Science in Pharmacy from an accredited college of pharmacy or university Registered with Singapore Pharmacy Council
