Staff Nurse (Home Care)

- Ensure that nursing care services are delivered safely, competently, and tailored to the individual needs of patients, while maintaining high standards in documentation, treatment, patient monitoring, and infection control practices.
- Provide skilled management of complex patient care, utilizing clinical expertise, knowledge, and critical thinking to deliver effective care.
- Continuously monitor, reassess, and evaluate the patient's condition, develop and implement appropriate care plans, and offer support to families in managing the challenges of advanced disease.
- Coordinate patient care by collaborating with advising physicians, directly providing care, assessing patient conditions, and acting as the primary liaison between families and medical teams.
- Foster positive communication and build rapport with patients and their families or caregivers, while assessing home care needs, including training requirements and the provision of necessary equipment and aids for the patient’s well-being.
- Ensure the availability of required equipment and resources to meet patient care needs, allocating items as necessary.
- Follow up to guarantee continuity of care and monitor adherence to treatment plans.
- Administer nursing care, including medication management and wound dressing, as required.
- Maintain accurate and up-to-date documentation of patient conditions, including detailed records of referrals, inquiries, and attendance.
- Actively participate in Multidisciplinary Team meetings and collaborate with other teams to ensure seamless delivery of patient care and services.
- Implement safety measures to protect patients at all times.
- Comply with organizational and legal requirements related to the storage, handling, and control of drugs and medical supplies.
- Uphold confidentiality and discretion in all matters related to patient care.
- Ensure proper use and maintenance of equipment and promote the economical use of consumables and supplies.
- Handle public inquiries professionally and appropriately.
- Adhere to AH’s policies and procedures, while ensuring compliance with the Singapore Nursing Board’s Standards of Practice for Nurses and Midwives, as well as the Code of Ethics and Professional Conduct.
- Stay informed of current industry guidelines and continually develop new skills relevant to the role.
- Maintain high standards of cleanliness, organization, and workplace safety at all times.
