Reports To: Homecare Nursing Supervisor
Job Summary:
The Registered Nurse will deliver professional nursing care to patients in their homes, following the established care plan, ensuring the best possible outcomes. This role requires a nurse who is compassionate, detail-oriented, and able to work independently while providing high-quality healthcare services to a diverse patient population.
Key Responsibilities:
- Provide comprehensive nursing care to patients in their homes, including assessments, care planning, implementation, and evaluation.
- Administer medications, treatments, and other prescribed therapies, following protocols and physician orders.
- Monitor patients' conditions, perform regular evaluations, and report changes or concerns to the supervising physician or healthcare provider.
- Educate patients and their families on health conditions, care plans, and post-treatment care to promote health and wellness.
- Collaborate with a multidisciplinary team, including physicians, therapists, and social workers, to provide coordinated care.
- Maintain accurate and timely documentation of patient care, assessments, interventions, and outcomes in the electronic health record system.
- Ensure compliance with all health regulations and standards, including infection control practices, patient confidentiality, and safety protocols.
- Assist in developing and updating individualized care plans to meet the unique needs of each patient.
- Provide emotional support and guidance to patients and their families, addressing any concerns and ensuring a positive patient experience.
- Participate in ongoing professional development and training to enhance nursing skills and stay current with best practices in homecare nursing.
- Education: Bachelor of Science in Nursing (BSN)
- Licensure: DOH license (DHA or home country license holders can also apply)
- Experience: Min. 2 years
- Skills: Strong clinical skills, excellent communication and interpersonal skills, and the ability to work independently.
