Sunshine Coast Hospital and Health Service (SCHHS) is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south and Gympie in the north.

Key Responsibilities include:

• Provide support services to determine the priority needs of senior Queenslanders, including the assessment of aged care requirements and a summary of assessment findings, goals and recommendations.
• Undertake Home Support assessments for Commonwealth Home Support Program services to meet the ongoing needs of the older person.
• Identify clients who would benefit from a Reablement Focused Program and action referrals to the appropriate services.
• Develop a client centred Support Plan with the client and /or representative to meet their needs to enable them to live in the community.

Remuneration value up to $97 656 p.a., comprising salary between $79 963 - $85 591 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (AO3).
