PACE is a non-profit community support service organization that provides support services to adults with disabilities who are committed to living independent lives. PACE provides support services to more than 350 individuals who have a variety of disabilities. We operate 7 supportive housing sites where people live independently along with a Community Outreach Program that provides support services in people’s homes. PACE has been accredited with Accreditation Canada, since 2010. PACE achieved the highest rating of Accredited with Exemplary Standing in our last survey in May 2022.

Position Overview
PACE is looking for an energetic, self-directed and collaborative individual to join our Client Resource Team and assume the role of Client Intake & Assessment Facilitator. Reporting to the Senior Client Manager, Interdisciplinary and Enhanced Care, this newly created position will be client-facing and responsible for intake, transfer, and discharge; care assessment and case management with a focus on assisting clients in living independently and promoting quality of life.
What you will do:
The Client Intake Coordinator will:
- Facilitate Client Intake, Transfer & Discharge
- Assist Program Management with project vacancies by short-listing clients for interviews, setting up interviews and home assessments
- Conduct relevant intake assessments using different assessment tools as needed i.e., Barthel Index, Mayo Portland, Inter RAI
- Develop, document, and maintain client case notes, statistics, letters, acuity ratings,
- Maintain the waitlist for the acquired brain injury program and collaborate with ASAC and Toronto ABI Network regarding referral info.•
- Assists Program Management and Client Resource Facilitator by coordinating client transfers and discharges to and from different
- Coordinate Client Care
- Assess client requirements and makes determination of eligibility/suitability for services.
- Builds relationships with other healthcare providers, vendors, community partners to integrate services across the healthcare system to ensure continuity and quality of care while promoting the client's self-management and independence. •
- Help clients navigate the Health Care system.
- Provide Case Management
- Collaborate with Client Resource Facilitator through case conferences, conduct client visits and assist with hospital discharge as needed.
- Work with inters-disciplinary, multi-agency teams to navigate access to required services.
- Coordinates and participate in case conferences.
You will have a Post-Secondary degree/diploma in Social Work, Social Services or other related disciplines. You will also have a minimum of three (3) years’ experience in a human services organization preferably in a unionized setting with adults in the disability sector. An individual who is a certified Inter RAI assessor is preferred or required. You also are an excellent communicator and collaborator and have the ability to work in a changing environment. You have a passion for working with PACE team members including clients and community service providers and support the philosophy of independent living.
Other Information
The position is full time, 40-hour week, Monday to Friday; the role is hybrid; three days/week primarily at Head Office (Dufferin/Lawrence) and two days/week remote. Regular travel to other program sites/client homes within the GTA will be required
