Dietitian - Diabetes Educ HDH (98069)

The Registered Dietitian, Diabetes Educator is responsible for provision of nutritional care of adult and pediatric patients diagnosed with Diabetes. The dietitian provides consultation, assessment and education; promotes and fosters a culture of understanding and compassion in the management of chronic disease, and diabetic clients; participates in program development, implementation and evaluation; is responsible for participation in departmental, hospital and educational activities which are supportive of professional and corporate objectives. Adheres to professional obligations as required by the College of Dietitians of Ontario. The Dietitian is responsible for delivery of care consistent with the vision and values of the hospital and the standards of relevant professional organizations.

PRINCIPLE DUTIES & RESPONSIBILITIES:
  • Assesses patients requiring nutritional intervention that are referred to the Diabetes Education Management Center. Interprets and evaluates nutrition, health status, lab and medical data and provides appropriate treatment recommendations (e.g. dietary modifications, medication titration/adjustments).
  • Assists patient with insulin dose titration and provides counselling in accordance with medical directives.
  • Monitors and interprets blood glucose levels and laboratory values to assess optimal glycemic control.
  • Evaluates therapeutic outcome treatment, and maintains appropriate documentation related to all patients under his/her care.
  • Demonstrates use of adult teaching and learning strategies in developing and delivering education to patients.
  • Provides diabetes education to patients identified as having poorly controlled diabetes or newly diagnosed type 1 or 2 diabetes including such topics as self-blood glucose monitoring, insulin administration, use of oral agents, identification of hypoglycemia and long term complications.
  • Teaches patients how to use appropriate equipment, such as insulin pumps and blood glucose meters, to track and monitor diabetes
  • Proficient in troubleshooting and operating insulin pump therapies.
  • Monitors and interprets blood glucose levels and laboratory values to assess optimal glycemic control.
  • Provides telephone advice to patients or family members with diabetes and documents in the patient record.
  • Develops and maintains updated nutrition care plan, counsels and educates patients/ clients individually or in groups. Liaises with outside agencies/organizations for appropriate nutritional monitoring.
  • Refer patients to other health care professionals and make recommendations to consult other medical specialties.
  • Acts as an integral member of the multidisciplinary team in designated clinics and medical services.
  • Develops, reviews, updates, implements and evaluates educational material and programs for patients and family members

QUALIFICATIONS:
  • Baccalaureate degree in Food and Nutrition.
  • Completion of a Dietitians of Canada (DC) approved dietetic internship, or approved DC equivalent
  • Eligible for College of Dietitians of Ontario (CDO) membership and practice within the professional, legal, and ethical standards outlined for Dietitians in Canada.
  • College of Dietitians of Ontario, Dietitians of Canada Association.
  • Basic Cardiac Life Support at the Health Care Provider Level.
  • Experience in clinical nutrition, consultation, assessment and education for persons with Diabetes.
  • Familiarity with the nutritional care of patients diagnosed with Diabetes.
  • Commitment to provide a positive patient experience with adequate support and positive patient outcomes.
  • Ability to work effectively as a team member.
  • Excellent written and oral communication skills
  • Commitment to standards of excellence and accountability
  • Good working knowledge of relevant legislation including:
    • Personal Health Information Protection Act,
    • Regulated Health Professions Act (RHPA)
    • Public Hospitals Act
    • Copyright legislation
    • Occupational Health and Safety Act.
  • Excellent working knowledge of Microsoft Office including Word, Power Point, and Excel.
  • Demonstrated good interpersonal, communication, and organization skills.
  • Capable of coping with a physically demanding workload.
  • Two years recent work experience as a Clinical Dietitian in an academic health sciences centre.
  • Experience in an ambulatory care environment
  • Certification as a Diabetes Educator (or willingness to complete within two years of hire).
  • Proficiency in speaking and comprehending both English and French.
  • Proven ability to attend work regularly.
  • Satisfactory criminal reference check and vulnerable sector search required.
