Integrated Youth Hub Clinical Coordinator

The Hub Clinical Coordinator uses in-depth knowledge, experience, and expertise within their professional discipline to provide clinical leadership and to promote a culture of respect, continuous learning, and professional development within their hub. They administer, oversee, and coordinate allied integrated health services offered by the NB-IYS (Integrated Youth Services) Wellness Hub, ensuring the seamless delivery of its core service pathways. This leader promotes recovery, youth centered care, and family inclusiveness to promote and advance excellence in clinical practice that is youth-centered and honors diversity of persons. They inspire and practice excellence.
This role provides clinical leadership for the development, delivery, and evaluation of services, outcomes and care planning for the effective utilization of resources through a systems and integration perspective. The Clinical Coordinator acts as a clinical resource to staff, students, and inter-professional team members, facilitates effective problem solving, and promotes collaboration. This position establishes priorities and coordinates growth of core and ancillary services, and the service assignments and schedules for clinical services. The role will identify learning needs and interests of staff, provide educational and orientation programs, coach and mentor, conduct clinical support oversight, debriefing, risk management, and provide formal feedback.
As the hub Clinical Coordinator they interpret, implement, and monitor policies, procedures, and standards related to client care in all environments. They collaborate with interdisciplinary team members, as well as other NB-IYS hub sites and health and community providers to ensure timely and seamless delivery of services. They identify, coordinate and promote evaluation, research, and quality improvement initiatives. The hub Clinical Coordinator participates in team and/or local committees and internal and external conferences. This role also is an integral part of a provincial network through a clinical community of practice consisting of clinical leads from across NB-IYS. The balance among domains of work will vary depending upon the stage of implementation and needs of the youth, the practitioners, the care settings, and the communities of the regional hub.
Key Duties and Responsibilities:
- Provides clinical guidance and expertise, mentoring, coaching and consultation to the center’s interdisciplinary service providers, students, interns, and partners in the clinical care of young people and their families.
- Collaborates in problem solving clinical cases, providing and/or recommending interventions, programs, resources and literature to promote continuity of care and effective treatment and resource utilization.
- Supporting team members in areas of peer support, youth care work, care navigation, counselling, and other support services
- In conjunction with Hub Operations Manager, oversees daily workflow for walk-in and drop-in services, clinical and non-clinical pathways of services, other hub allied health services and programs, including monitoring caseload allocation where appropriate.
- Provides clinical support and supervision for walk-in counselling when applicable, with an emphasis on evidence-informed and culturally responsive practice, discussing client care issues, and developing linkages and partnerships with other facilities, agencies, and support groups.
- Collaborates with physicians and/or nurse practitioners, the inter-professional team, and other services to develop, implement and evaluate comprehensive care plans and interventions for youth and their families using systematic assessment, best practices, and evidence-based standards, involving the youth and, where appropriate, the family in the context of their community.
- Emphasizes the importance of providing integrated services for clients. Acts as a resource for mental health and substance use issues. Promotes collaboration between staff and families to highlight client concerns such as access to primary care, education, employment, housing and income support.
- Participates in committees, task groups, Communities of Practice, continuing education and/or community initiatives by methods such as providing mental health and substance use representation and input from a clinical perspective; sharing up-to-date information on programs, services and referral processes; acting as an advocate for the goals and objectives of the clinical team; facilitating the development of clear, efficient service and referral pathways; and promoting optimal support for clients and their informal support network to improve access, integration and coordination of services.
- In collaboration with the Hub Operations Manager, engages in ongoing program planning, development, evaluation and quality improvement activities that facilitate team goal setting, problem solving, conflict resolution and collaboration by methods such as assessing resources, collecting data, implementing client outcome measures, monitoring variances to expected outcomes and analyzing outcomes data. Reviews feedback, standards, procedures and protocols to determine opportunities for quality improvement and provides input and/or recommendations for improved service delivery and to reduce gaps and barriers in services.
- In conjunction with the Hub Operations Manager, to support operations, supports the selection process and hiring of new staff, taking into consideration the needs of the collaborative team environment. Provides orientation and training for new staff as needed. Provides input to the program manager on performance evaluations for designated staff. Identifies and supports learning and development needs of staff.
- Performs other related duties as required
- Degree in social work, with 5-10 years of related experience in clinical specialty area.
- Experience working in a multi-disciplinary team.
- Experience working directly with youth.
- Experience with effective stakeholder mobilization, leading teams and/or initiatives.
- Experience with developing policies and procedures and designing and delivering community programs.
- Primary care experience (optional).
- Satisfactory criminal record check and clear vulnerable sector search.
- Comprehensive understanding of mental health and substance use issues among adolescents, young adults, and families.
- Demonstrated experience in supervisory processes, clinical supervision models, and/or the ability to assess learning needs, teach, coach, and provide clinical support.
- Demonstrated therapeutic skills in case management, individual, family, and group therapy for clients with diverse mental health and substance use illnesses.
- Demonstrated understanding of trauma-informed and culturally safe care principles.
- Familiarity with team-based primary care and relevant Acts, regulations, policies, and certification requirements (e.g., Mental Health Act, Freedom of Information and Protection of Privacy Act).
- Strong listening and consensus-building skills to facilitate communication and decision-making in interdisciplinary settings.
- Capability to work independently and collaboratively in multicultural environments as part of an interprofessional team.
- Ability to manage crisis situations and provide leadership to staff in the management of client crises.
- Knowledge of NB's mental health system, community organizations, and resources.
- Excellent communication skills, including speaking, listening, and written communication.
- Strong interpersonal skills, flexibility, and effectiveness at individual, program, and system levels.
- Highly motivated and passionate about mental health and anti-oppressive work.
- The workweek is 37.5 hours.
- Flexible work schedules may be required to meet operational requirements.
- Travel within the province and the service region is required.
- Access to reliable transportation is necessary.
- Work in various settings or in partnership with other health agencies is expected.
IYS is transforming youth mental health and substance use services in communities across Canada and helping to ensure that young people have equitable access to the range of services they need, when they need them. It’s a dynamic pan-Canadian and international movement that aims to build effective, youth-focused and integrated services for mental health, substance use and related issues. IYS is founded on key principles that can be implemented in different models across jurisdictions and types of communities. Principles of IYS include:
- Youth and family participation in the design and implementation of services.
- Integration of services (mental health, primary care, addictions, counselling, peer support, hospitals, education, social services, including housing, justice, employment) across service providers.
- Continuity of services for youth and young adults (e.g., 12 to 24 years) – this can include integration of child and adult services that focus on this age group.
- Clearly identified, easily accessible points of care in the community that provide rapid access to services – no service refusal, no referral needed, and access to high quality, youth-appropriate, evidence-informed mental health services (could be through integrated service hubs, safe spaces, schools, universities, etc.)
- Ensuring equitable access, including for vulnerable populations of youth, and culturally appropriate care.
- Stepped care models – evidence-informed, appropriate levels of care, at the right time by the right provider to ensure efficient utilization of services.
- Incorporation of online portals and other technologies that facilitate alternative access to information and services.
- Strong research and evaluation to guide continuous quality improvement and measurement of the impact of the transformation of services.
