ROLE PURPOSE

This position will provide outreach support to client(s) referred to Connective by our funders and community partners. This role will assist with improving housing stability through an integrated service provision approach and aid vulnerable individuals with maintaining housing in the private rental market.

STATUS AND CLASSIFICATION

Unionized. This position is open to applicants of all genders.

HOURS (20 Hours Per Week)

Monday to Friday, 1000 - 1500 (flexible according to management needs).

REPORTING AND WORKING RELATIONSHIPS

  • Reports and is accountable to the Program Manager or their designate
  • Participates as a member of Connective team to ensure quality and continuity of services to clients
  • Liaises with staff and volunteers of community and government agencies as required
  • Liaises with Connective staff as necessary
  • Accepts clinical direction from the care team
KEY ACCOUNTABILITIES

  • Conducts referral consultations based on criteria, urgency and other factors to aid with a successful transfer or referral process
  • Works with the client, research background material, and seeks the input of other professionals and family members, to provide the assessment that will ascertain needs and postulate intervention possibilities
  • Works with the client to consider issues and creates a mutually agreed intervention and support plan that has measurable short- and long-term goals
  • Ensures aspects of daily living, including but not limited to housing, nutrition, clothing, hygiene, leisure lifestyle and access, employment, and education are addressed in planning
  • Provide coordinated case management services, physical health, mental wellness and substance use supports, social, emotional and community supports, personal care and personal living supports, food security supports, indigenous cultural supports
  • Provide direct nonclinical supports to recipients and work with community partners including the health authorities
  • When and as needed, provides a wide range of interventions to support the client in addressing and accomplishing the established goals
  • Interacts with other professionals, family members, and organizations to ensure services, admissions, and treatments are in the client’s interest and supportive of long-term goals
  • Data collection and input through monthly reports and analysis of case notes
  • Required to drive between various sites to meet with clients and community partners, as well as transport individuals between sites or to appointments within the community
  • Participates in staff meetings as requested by the Program Manager
  • Other duties as subsequently negotiated
WORKING CONDITIONS

  • Functions independently, while remaining a crucial member of the care team
  • Adheres to Connective policies and procedures
  • Is available to work flexible hours as needed, which may include some evenings/weekends, as determined by program/client needs and schedules
  • Is a member of CUPE Local 1936-11
  • Receives benefits and works within the working conditions as outlined in the collective agreement and personnel manual
EXPERIENCE AND SKILLS

  • Graduate with a Diploma or Certificate in relevant field with one year of related experience
  • Demonstrate a clear ability to respond well in crisis situations
  • Experience with clients with a history of conflict with the law, developmental disabilities and/or spectrum disorders, interaction with social agencies and experiencing psychiatric/personality disorders
  • Understanding of providing service to people with different learning abilities
  • Extensive knowledge of community services in Northern British Columbia
  • Experience in assessment, case/care plan development and implementation
  • Demonstrated an ability to deal with potential violence
  • Demonstrated an ability to develop programs tailored to the particular diverse needs of the individual
  • Commitment to Indigenous Truth and Reconciliation, cultural safety, and decolonization practices
  • Excellent written and verbal communication skills in English
REQUIREMENTS

  • Ability to successfully complete a Criminal Records Check
  • Current Emergency First Aid Certificate
  • Class 5 driver’s license
  • Personal transportation that is in good repair, insured for business up to $1,000,000 for liability, and which can be used to transport clients
  • Proof of vaccination for communicable diseases may be required as mandated by Government, Health Authority, and/or funder requirements.
COMPENSATION

As Connective is a unionized environment, the starting wage is $27.54 per hour. Plus benefits enrollment as per the Collective Agreement.

Important: Candidates must be legally authorized to work in Canada, observe their weekly hours limit if under a temporary or study visa, and provide proof of eligibility if selected for the role.
