Pharmacist

Responsibilities:

Manage pharmacy operations including medication reconciliation, reviews, proper medication storage, and monitor inventory and procurement of all drugs and related items
Check prescriptions to ensure error-free and safe medication for patients
Dispense according to the prescription
Plan, implement and maintain procedures for mixing, packaging and labeling pharmaceuticals according to policy and legal requirements to ensure quality, security and proper disposal
Collaborate with other health care professionals to plan, monitor, review and evaluate the quality and effectiveness of drugs and drug regimens
Handle and manage patients’ self-brought medications
Answer questions about medicines from HKACS nursing home and community drug programmes
Suggest and formulate guidelines to ensure a good medication system used in the nursing home
Supervise the work of dispenser
Manage records of pharmacy, e.g. Dangerous Drug Register, invoice, etc.
Carry out/participate in continuous quality improvement programmes and audits as appropriate

Requirements:

Bachelor degree or above in Pharmacy
A certificate of Registration and a valid Practising Certificate issued by the Pharmacy and Poisons Board, Hong Kong
2 years relevant work experience as a pharmacist
Good interpersonal, communication skill and good team player
Self-motivated, flexible, passionate in community engagement and patient-centred care
Solid experience in using the Safe Med Medication Management System (SMMS) will be an advantage
Willing to work overtime and/or weekends if required
Provide emergency and/or contingency support on medications clinically if necessary

Successful candidates will be offered a renewable contract term appointment with up to 10% of total basic salary as contract-end gratuity upon completion of contract with satisfactory performance.
