The Hong Kong Society for Rehabilitation (hereunder as ‘HKSR’) is a pioneer in
rehabilitation dedicated to providing holistic and personalized services to empower persons
with disabilities, chronic illnesses or challenges of aging to lead fulfilling lives, and to

JCECC “Life Rainbow” End-of-Life Community Care Project is funded by The Hong
Kong Jockey Club, with the aim to enhance the quality of life of persons with late-stage
chronic illness and their families by using a collaborative partnership approach.

Project Officer (Part-time)
(Ref: O(P)(PT)_ECC_OCT24)

Key responsibilities:
Assist in psychosocial-spiritual care activities led by social workers including but not
limited to wish fulfilment activities, legacy activities and family outings
Provide practical support including escort services, patient-sitting service, equipment
loan, home-making service and in rare occasion personal care
Coordinate on evaluations and data input
Coordinate and prepare for reports on administration and budget aspects
Responsible for public education events and resource development

Requirements:
Degree holder or above
At least 1 year experience in work with administrative support and coordination
Experience to work with frail elderly or similar target group is needed
Knowledge in chronic disease management or vocational rehabilitation, mutual
support service, volunteer service and e-service delivery
Good inter-personal, communication and time management skills
Mature, self-initiative and innovative
Good team player in a multidisciplinary team
Good command of both Chinese and English
