Duties :

• To provide palliative and end-of-life nursing care for children / adolescents with life-threatening and life-limiting illness and their families in the community
  
• To work together with the healthcare teams in hospitals and join regular case conferences to provide children's palliative care services
  
• To collaborate and develop relationships with a wide range of healthcare workers, schools, agencies and other NGOs to provide a continuum of care services for patients and their families
  
• To perform standby duties, work irregular hours to support families in need, and join activities organised by CCF in non-office hours at times
  
• To provide outreach services in hospitals and community

Requirements :

• A Certificate of Registration (Part I) holder with a valid Practising Certificate issued by the Nursing Council of Hong Kong
  
• At least three years of post-qualification experience preferably in the areas of paediatrics
  
• Excellent command of written and spoken English, Cantonese and Mandarin
  
• Passionate in serving paediatric patients with medical complex and palliation conditions and their families
