Options Case Management, LLC is an Indiana Pathways Service Coordination Provider as well as a Health & Wellness and TBI Medicaid Waiver Case Management Provider serving all 92 Counties in the State of Indiana. We are currently hiring Hybrid-Remote/Home Based/Contract Case Managers in Bartholomew and surrounding Counties in Indiana. Home visits in Indiana are required so you need to live in Indiana.
The Care Manager (CM) and Service Coordinators assists and enables individuals/participant to remain in the least restrictive environment while maintaining the greatest amount of independence and human dignity; assists the participant with access to and coordination of services to enable the individual to receive arrange of appropriate services in a planned, coordinated, efficient and effective manner. Promote and support the participants preferences, values, and the right to self-determination.
Minimum Qualifications:
*The minimum of a Bachelor's Degree from an accredited institution, in a Human Services field such as Psychology, Social Work, Nursing or related.
*Demonstrates strong critical thinking, problem solving skills and flexibility in relationship to job requirements; Computer skills sufficient to enter information electronically and use Options Case Management website and email, i.e. familiarity with Microsoft Office programs.
*Effective communication (oral and written) and interpersonal skills with respect for others including individuals, co-worker and other professionals within adverse work environment.
*Available to meet the needs/requirements of staff, co-workers and individuals during Options Case Managements standard business hours; reliable transportation and ability to travel as necessary. Valid driver’s license and car insurance as required by state law.
