Reporting to the Integrated Operations Centre Manager, this role co-ordinates clinical care and discharge planning for patients across adult services who are identified as at risk of extended length of stay.
The position will work in close partnership with both clinical and operational team members and function as a key member of the interdisciplinary team to optimise safe and timely patient flow and the development of robust discharge pathways which directly contribute to achieving shorter stays in Emergency Department.
The role is fixed term to enable evaluation of the effectiveness of the role against key measures and to ensure the role can evolve as required.
He korero mōu - About You
You are experienced with systems and process relating to complex discharge planning and co-ordinaiton and have recent acute care experience. You also have a clear quality lens and can work proactively as part of the team to resolve and break down identified barriers to discharge. You have a proven ability to engage and work together with the wider clinical teams and never give up in striving for improvement in positive discharge outcomes for patients and their families/whanau.
