Social Worker - Early Supported Discharge

Te Pae Hauora o Ruahine o Tararua Mid Central (Mid Central) is part of Te Whatu Ora, Health New Zealand and provides healthcare services to over 180,000 people across the Manawatū, Horowhenua and Tararua regions. With a large hospital campus in Palmerston North, it also provides key health care and support services to neighbouring communities.
The Opportunity
Are you a skilled and motivated clinician ready to embrace a new challenge? We are launching an innovative Early Supported Discharge (ESD) service, and we're looking for dedicated Allied Health Clinicians to join our team from the ground up!
This is a unique opportunity to shape a brand-new service, designed to support patients in transitioning from hospital to home. As part of our dynamic ESD team, you'll work in a fast-paced environment where your expertise will be vital in enhancing patient recovery through early discharge and home-based rehabilitation. We have a number of positions so can consider full time or part time hours.
We are looking for individuals who:
- Have an Enthusiastic, energetic, resilient, with a ‘can do' attitude
- Have the ability to work effectively within a team
- Holds New Zealand registration with the appropriate registration board and a current practising certificate
- Are driven with a strong desire to help clients and whānau achieve their goals
- Have effective written, oral and interpersonal skills with an ability to communicate with people from a wide variety of backgrounds and cultures.
- Brings strong organisational and time management skills
- Holds a full New Zealand clean and current driver's licence
- Having experience working in a similar environment would be an advantage but not essential.
