- Perform detailed assessments of patients' psychosocial aspects to determine their needs, strengths, and available support
- Lead psychosocial care planning with multidisciplinary healthcare teams, ensuring alignment with patient goals and resources.
- Act as a liaison between patients, families, and medical staff, ensuring that care plans are clearly communicated and followed
- Oversee care coordination, ensuring seamless transitions for patients between home, hospital, or other care environments
- Identify and connect patients and their families with community resources, including financial aid, home improvements, transportation, and local support groups
- Educate patients and their families about healthcare systems and policies to help them make informed care decisions.
- Provide supervision and mentorship to junior social workers and interns
- Participate in supervision, case conferences, and multidisciplinary team meetings to enhance service delivery.
- Degree in Social Work or Master's Degree in Social Work or equivalent.
- Registered with Singapore Association of Social Work.
- Minimum 5-7 years of relevant work experience, preferably within healthcare settings
