DESIGNATION : Social Worker

RESPONSIBILITIES
About the Job
The Social Worker is responsible with working with clients and their family in the day centre(s) to provide psychosocial support and interventions for persons receiving centre and home-based services.
Key Responsibilities

• Work closely with clients and family with regards to their admissions and discharges to the day centre services.
• Assess client's support system/network and household condition as part of assessment and care planning.
• Provide case management services and facilitate appropriate casework interventions.
• Work closely with multidisciplinary team and other healthcare professional to discuss and implement holistic care plans for clients.
• Liaise and make appropriate referrals to community service providers to meet the needs of the clients and their families.
• Maintain accurate and current documentation of casework.
• Conduct review of household financial information, financial stressors and to assist client and their families to apply for appropriate financial assistance

QUALIFICATIONS
Degree/Graduate Diploma in Social Work
Preferred Candidate Attributes

• Preferably at least 3 years of experience as Social Worker in social service sector.
• Experience and knowledge in the healthcare/eldercare/community care sector is a plus.
• Good interpersonal and communication skills with persons of all levels.
• Ability to work independently as well as being a team player.

OTHER INFORMATION
Working Hours
Under normal circumstances the hours of duty will be as follows:
8.30am to 5.45pm (Monday to Friday, 45 minutes Break inclusive)
