**Role Overview:**
We are seeking a passionate and organized Care Coordinator to join our team. In this role, you will play a vital part in managing care services, ensuring that our clients receive the highest quality support. You will work closely with clients, families, and care staff to develop, review and implement effective care plans.
**Key Responsibilities:**
- Assess clients’ needs and create personalized care plans in collaboration with clients and their families.
- Coordinate and schedule care staff to ensure optimal service delivery.
- Monitor the quality of care provided and make adjustments as needed.
- Maintain accurate and up-to-date client records and documentation.
- Communicate regularly with clients, families, and healthcare professionals to address any concerns or changes in care.
- Provide support and guidance to care staff to enhance their performance and job satisfaction.
- Spot checks and review of care staff perfromance
**Qualifications:**
- Previous experience in care coordination or a similar role in domiciliary care.
- Strong organizational and communication skills.
- Ability to work independently and manage multiple tasks effectively.
- A background in healthcare or social care is advantageous.
- Proficient in using care management software and Microsoft Office.
