**Job Title: Care Coordinator (Domiciliary Care)**
**Location:** Hybrid in Bedford
**Salary:** from £12.50- £14.0 depending on experience
**Hours:** Part-time

**Role Overview:**

You will work closely with clients, families, and care staff to develop, review and implement effective care plans.
**Key Responsibilities:**
- Assess clients’ needs and create personalized care plans in collaboration with clients and their families.
- Coordinate and schedule care staff to ensure optimal service delivery.
- Monitor the quality of care provided and make adjustments as needed.
- Maintain accurate and up-to-date client records and documentation.
- Communicate regularly with clients, families, and healthcare professionals to address any concerns or changes in care.
- Provide support and guidance to care staff to enhance their performance and job satisfaction.
- Spot checks and review of care staff perfromance
**Qualifications:**
- Previous experience in care coordination or a similar role in domiciliary care.
- Strong organizational and communication skills.
- Ability to work independently and manage multiple tasks effectively.
- A background in healthcare or social care is advantageous.
- Proficient in using care management software and Microsoft Office.
