As a care Rapid Responder, your key focus areas will be delivering the highest quality levels of home care and support to our service users. We are proud to support 12,000 carers and their families every year. Our vision is of a world where carers have choices.

About you
This is a fantastic opportunity for you to work within our community in a small, highly effective team in Norwich, providing care and support.
· You are passionate about making a difference in your community.
- You will have previous experience in social care with a minimum of NVQ/QCF Level 3 in care.
- As part of our wider care team you will also be required to support our emergency cover service overnight which is paid at an hourly rate and includes a retainer payment
- A knowledge of risk assessments & care planning is essential
- You will need a full valid driving licence, access to a vehicle and a kind heart!
- Willingness to work a variety of hours, including alternate weekend. As part of our wider care team you will take part in our out of hours rota.

- Permanent post working 37 hours per week.
- Be part of a responsive team providing support to customers and their families.
- With your knowledge & experience you will be ensuring people who use our services receive safe, caring, responsive, effective care & support.
- Provide care support in the community via our home care service, including complex cases.
- To play a proactive role in signposting patients and their families to other relevant support services (both internal and external, both statutory and non-statutory).
- Complete contingency risk assessments, assessments and risk assessments for new and existing customers and emergency packages of care to maintain a ‘rapid response’ to customer need and ensure high quality support
- Ability to work as part of a team and under own initiative.
- Ability to work under pressure and cope with change and conflicting priorities.
- Seeks continuous improvement.
