About The Role
Social Workers within this service area can work flexibly between home and work to meet the needs of the service whilst recognising the benefits that a hybrid model of working can bring.
We are looking for confident and skilled Social Workers to work in our Leaving Care service where you will work closely with children in care and their carers to progress their care plans and to support our children to transition to adulthood. Our objective is to deliver the best outcomes for children and young people.
Reporting to the Team Manager, your role will consist of completing direct work with young people to build effective relationships, ensure they have a good pathway once they leave care, ensuring their voice, wishes and views are central to the planning and delivery of work.
You will lead and coordinate multi-agency partnerships, plans, interventions, and meetings to ensure children and young people’s permanence plans are being progressed and their holistic needs are being met. Within your team, you will play a central role in achieving positive outcomes for children and young people in the care and those leaving care. As a Social Worker you will ensure that the young people are at the heart of your decision making.
Key Responsibilities
- You will ensure young people have a clear pathway once they leave care and support their transition to adulthood.
- You will work to achieve permanency and outstanding outcomes for children and young people through relationship-based practice.
- Building strong relationships to support the development of good quality pathway plans which you will review and monitor ensuring the needs of our young people are addressed and met.
- Encourage and empower young people to participate in all decision-making processes affecting their lives
- Provide information, advice, planning and preparation and holistic support to meet young people’ developing needs, particularly in health, housing and post-16 careers
- Writing high-quality reports and assessments and analyse information effectively.
- Care plans will be implemented, reviewed and remain SMART and in line with the children and young people’s views, ensuring a clear focus on their needs and permanence.
- Completing direct work, including life story work to ensure the children and young people’s voice is heard and included in all decision making.
- Hold a DIPSW or equivalent qualification and are registered with the Social Work England as a qualified social worker.
- Have a full driving license and car available for work (unless a disability prevents this).
- Strong interpersonal skills.
- Experienced and expert in managing the delivery of quality outcomes for children and young people.
- An ability to build strong relationships with children, their families, colleagues and partner agencies.
- An ability to write high-quality reports and assessments and analyse information effectively.
- £3000 retention bonus per annum
