Staff Nurse - nights

An exciting opportunity has arisen for a Staff Nurse to work at Mountbatten Hospice in Hampshire. As a member of the team there will be opportunity to work a variety of shifts; we are particularly seeking Registered Nurses to work night shifts in either our Inpatient or Community settings.
**This is a BANK ROLE - Zero Hours Contract***
Main Duties of the Job
You will have excellent opportunities for training and development to enable you to expand your skills within Palliative and End of Life Care. You will be responsible for providing symptom management and complex end of life care to patients living with life limiting conditions. You will be able to make a real difference to peoples lives every single day supporting our patients and their families. You will also be responsible for the provision of delivering high quality and timely care for patients and invaluable support to the patients family.
About You
You will be a Registered nurse with current NMC registration employed by Mountbatten, who is able to demonstrate a commitment to their own CPD. You will have excellent clinical skills and experience and be able to work under supervision and on your own imitative to provide the highest standards of care and support to your patients and their families. You will have good communication skills, with sound experience of symptom management, you understand how to look after the patient and their family within the palliative field of care.
Overview of Role - IPU
To ensure the highest standards of individualised care for patients with life threatening diseases including providing support to their families and carers in a safe, friendly and efficient environment.
To communicate with patients, families and carers in a sensitive and empathetic way maintaining effective communication with multi-disciplinary team members. To work under the supervision and guidance of nurses and medical teams.
Although based on our Inpatient Unit, post holders may occasionally be required to work in other teams to ensure adequate staffing levels and to gain experience of the wider hospice delivery of care (e.g., Community Setting and Hazel Centre)
Key Tasks
To provide the highest standards of care to patients
To lead on and coordinate assessment, planning and evaluation of individualised patient centred care
To provide a holistic approach to all care provisions provided that is both sensitive and responsive to the patients ever changing needs
To participate in the checking and administration of prescribed drugs (including controlled drugs) and other substances in accordance with Mountbatten policy
To provide out of hours specialist advice by telephone to patients, carers and allied professionals as required, alerting them to any significant matters
To maintain appropriate written/computer records in all areas of clinical practice.
To liaise with medical staff to implement treatment and care, ensuring that medical instructions are carried out, monitoring and reporting effectively
Overview of Role - Community
The Associate Clinical Practitioner role will contribute to the provision of specialist palliative and end of life care in the Community, being based at Mountbatten Hampshire. Under the supervision of the Locality Manager, they will work autonomously and as a member of the Community Palliative Care Team influencing the delivery of ongoing patient care.
Key Tasks
To develop and maintain communications with patients, carers, other professionals and volunteers
To work collaboratively, building relationships with other members of multidisciplinary team and outside agencies to ensure individualised care is provided in a timely and responsive way
To provide advice by telephone to patients, carers and other professionals, as required from the Community Care Coordination Centre
To give and receive accurate reports and changes in patients condition, linking with key clinical staff where required, to facilitate comprehensive and safe patient care
To maintain accurate and contemporaneous records in all areas of clinical practice
To provide a rapid response visit to a patients home when required, undertaking a full clinical assessment of need, prioritisation and delivery of care
To undertake joint clinical assessments with other professionals and liaise as appropriate regarding ongoing care.

Pay: £18.22-£18.64 per hour
