How You'll Make a Difference:
Arkos Health is seeking a LPN Care Manager. The LPN Care Manager will be responsible for working with patients and their families to reduce emergency department visits and admittance to the acute facility while improving our Medical Loss Ratio (MLR). This role will collaborate interdepartmentally to adhere to state and federal regulations. The LPN CM will handle member engagement for proper nursing assessment and provide transitional and intensive care management services for the entire network and works collaboratively with the health plan, leadership, department coordinators to ensure the highest quality of patient satisfaction is met.


Essential Duties and Responsibilities:

• Conduct thorough Clinical and Social Assessment (CSA) of members enrolled in Intensive Care Management
• Develop personalized care plans in collaboration with healthcare providers and the interdisciplinary care team, update and revise as needed
• Maintain case load based on the needs of the client and/or contractual obligation
• Conduct visits at a minimum of every 2 weeks for assigned patients to monitor member’s health status and optimize health outcomes. May conduct increased face to face or telephonic visits based on the needs of the patient and/or contractual obligation
• Collaborate with healthcare providers to ensure seamless coordination of care; including obtaining referrals to other interdisciplinary services (Home Health, Palliative Care, Social Services) to help prevent further need for acute care utilization
• Complete medical reconciliation for all transitional care patients within 30 days of initiation of TCM contact
• Engage with Arkos Health leadership and contracted providers to resolve changes in patient status, complaints, or complications though participation in scheduled ICT meetings
• Work towards reducing acute visits (ED, inpatient, SNF) and associated costs by providing timely evidence-based interventions and education to members
• Collaborate with the Integrated Care Management (ICM) team to implement processes to prevent members from falling through the care caps and improve the Medical Loss Ratio (MLR)
• Ensure continuity of care by addressing potential barriers to adherence and empower members to actively participate in their healthcare journey
• Maintain accurate and up-to-date electronic health records for each member
• Perform other duties as assigned to support the overall effectiveness of the organization

Qualifications and Skills:

• Current, unencumbered, LPN License/ and graduation from an accredited LPN program
• Previous experience in care management, leadership and population health
• Current BLS/CPR certification
• Current TB test and maintain a negative test annually, Hep B vax/titer proof (Company does offer/or may sign a declination), Current flu shot (or sign waiver/follow mask policy).
• In-depth knowledge of health care services, nursing process and documentation
