What your day will look like....
You'll work closely with a diverse team to assess and develop tailored care plans for patients, engaging with them and their families to empower informed health decisions. Utilising your critical thinking and advanced assessment skills, you'll streamline discharge processes and enhance health literacy while contributing to quality improvement initiatives. Your commitment to mentorship will also help foster a culture of learning within the team.
