Job Title: Registered Nurse (Documentation Focus) – Temporary 3-Month Contract
Location: Bankstown, NSW
We are currently seeking a temporary Documentation Registered Nurse (Documentation RN) for a 3-month contract with one of our aged care clients.
Job Description:
We are looking for an experienced Registered Nurse with a strong background in clinical documentation within the aged care sector. As a Documentation RN, you will play a vital role in ensuring the quality, accuracy, and compliance of resident records. Your role will focus on comprehensive assessments, care planning, and coordinating case conferences to ensure that the needs and preferences of residents are met with the highest standards of care.
Key Responsibilities:
- Assessment and Care Planning:
- Conduct detailed assessments of new and existing residents to identify care needs, strengths, and potential risks.
- Develop and maintain individualized, person-centered care plans that address the physical, emotional, and social needs of each resident.
- Regularly review and update care plans in consultation with residents, families, and the broader clinical team to ensure they reflect current health conditions and personal preferences.
- Clinical Documentation Management:
- Ensure that all clinical documentation, including assessments, care plans, progress notes, and incident reports, are accurately completed and comply with aged care quality standards.
- Monitor the accuracy, quality, and timeliness of documentation by other clinical staff, providing training and support as needed to maintain best practices in record-keeping.
- Case Conferences:
- Organize and lead case conferences with residents, their families, and interdisciplinary teams to discuss care plans, address concerns, and agree on shared goals for resident care.
- Document case conference outcomes, ensuring follow-up actions are clearly recorded and communicated to the relevant care team members.
- Compliance and Quality Assurance:
- Maintain thorough knowledge of relevant legislative and regulatory standards (such as the Aged Care Quality Standards) and ensure all documentation practices meet these standards.
- Conduct regular audits of documentation to ensure compliance with standards, quality assurance policies, and facility protocols.
- Interdisciplinary Collaboration:
- Work closely with the facility’s multidisciplinary team to facilitate high-quality, coordinated care for residents.
- Act as a resource for other nursing and care staff regarding documentation standards and best practices, providing guidance, mentoring, and support where needed.
- Family and Resident Liaison:
- Serve as a key point of contact for residents and their families regarding documentation, care planning, and clinical updates.
- Address any queries from families or residents about their documentation, ensuring transparency, trust, and understanding.
- Education and Training:
- Provide documentation-related training and ongoing support for nursing and care staff, ensuring that all team members understand the importance of accurate and timely documentation.
- Continuous Improvement:
- Identify and implement improvements to the documentation process, aiming to streamline workflows and enhance the overall quality of resident care.
- Registered Nurse with AHPRA Registration: Must hold a valid, unrestricted AHPRA registration as a Registered Nurse in Australia.
- Experience in Aged Care: Demonstrated experience working in aged care, with a strong understanding of the aged care quality standards.
- Documentation and Clinical Assessment Expertise: Prior experience in documentation, care planning, and assessment in an aged care setting is essential.
- Strong Communication Skills: Ability to communicate effectively with residents, families, and interdisciplinary teams.
- Attention to Detail: Meticulous attention to detail with an understanding of the importance of accurate, legally compliant clinical documentation.
- Interpersonal Skills: Empathetic, approachable, and patient-centered, with a focus on delivering high-quality care and clear documentation.
