Registered Nurse (Home Care, Chinese speaking)

About the role
- To provide appropriate nursing care, health counselling, screening and assessment in accordance with nursing care plan.
- To support a system of quality improvement through which outcomes of clients care may be measured. To contribute to collecting accurate health care information and maintaining accurate health care documentation as required.
- To communicate effectively with clients and other health care team members in accordance with appropriate protocol.
- To advocates values of the organisation through ethical behaviour, professionalism and cultural diversity.
- Coordinate and supervise the delivery of daily care services to the residents under your care to ensure service quality and standards.
- Works collaboratively with members of the multidisciplinary team in the planning, implementation, assessment and evaluation of care
- Applies an appropriate level of clinical judgement in the provision of care adhering to legal and ethical requirements.
- Assist the supporting staff in providing all required personal care to residents when necessary.
- Provide feedback or update clinical information on clients condition to carer/coordinator
- Order medication as assigned and when necessary
- Maintain appropriate documentation on clinical care plan delivered to clients.
- Supervise to ensure relevant documentations are appropriately maintained
ABOUT THE SUCCESSFUL APPLICANT
The successful candidates should possess but not limit to the following:
Essential:
- Current AHPRA registration and a minimum of 1 year working in a home care setting
- Have knowledge of home care package levels and be familiar with Aged Care Quality Standards
- A high degree of computer competency including knowledge and competency in basic Microsoft applications
- Strong Assessment and Documentation Skills
- Good written and oral communication skills in both English and Chinese
- Demonstrated initiation and creative problem solving
- Ability to balance task and people focus/demonstrated customer service focus
- Evidence of good team building and interpersonal skills
- Demonstrated knowledge and application of WH&S, EEO and anti-discrimination legislative requirements
- Current Full Driver Licence
- National Police Clearance
- Australian Citizenship / Permanent Residency.
Specialised experiences in the following areas are highly preferred:
- Dementia Care: Assessment and care planning, behaviour support experiences
- Palliative Approach: Understanding palliative care in community-based sector and care planning
- Depression Awareness: Experience in Assessing and developing care plans for clients with depression
- Wound Care Knowledge: Up-to-date skills in wound care management
- Complex Care Management: Experience in enteral feeding, stoma care, and catheter care in client's homes.

Job Type: Casual
Pay: $51.85 – $64.83 per hour
Expected hours: 8 – 38 per week
