Are you the one we’re looking for?
We are seeking a dynamic, highly motivated Assertive Outreach Intensive Case Manager who demonstrates our values of compassion, accountability, responsiveness, and empowerment; to our clients, families, and each other.
Within a multi-disciplinary team, the Assertive Outreach Intensive Case Manager provides assertive outreach, support, and assistance to people living in tents, on the streets, and in precarious housing situations across Hamilton. Clients will have poor health and mental health, substance use problems with complex needs. The Assertive Outreach approach is consistent and persistent outreach to individuals who often will not want this contact. The position through outreach connects profoundly marginalized people to housing, primary health care, harm reduction and addiction supports, mental health services, and other supports that individuals will require.
This position is integrated into the Hamilton Police Services Social Navigation Team and uses a collaborative approach working with a variety of police, EMS, health care, mental health, housing, and social services to identify needs and connect individuals to supports. The position requires a large degree of independent work and high motivation in high-stress environments to act as a liaison and representative of Wesley in the broader community. The position works to enrich the quality of life of the client and promote their full and meaningful inclusion in the community.
This position is a unionized, full-time, temporary position. The contract ends on December 31, 2024. The rate of pay is $26.10 per hour. The position works 8:30 am to 4:30 pm shift, Monday to Friday.
Key Responsibilities:
- Working in a team-based environment composed of a sworn police officer, EMS paramedic, this position provides intensive case management services to assist in the active engagement of people who are profoundly marginalized, homeless, living in encampments and other unsafe situations, live in emergency shelters or are vulnerably housed in the wider community.
- Using a trauma-informed approach, provides assertive outreach services that focus on client needs, and individual choice, promotes client involvement, and is flexible in response to a client’s situation and needs
- Provides assertive outreach in the community; able to work rotation shifts during the weekend and flex hours which meets the needs of participants in the community.
- Uses evidence-based tools to assess the vulnerability of outreach clients in order to respond to those individuals that are most vulnerable and require a rapid response, the prioritization approach is to assist the person to exit homelessness, and reduce the likelihood of severe trauma, and illness.
- Attends daily Hamilton Police Services briefings and team meetings Document regularly in individual client files and logs, relevant/required information.
- Provide assertive outreach services by directly approaching people in their environment, who are homeless or at risk of homelessness who potentially are in poor mental and physical health, experiencing substance use issues, and/or experiencing profound deprivation
- Provide clients with the skills, information, and options they need to be empowered and make informed choices about their behaviour.
- Connect clients to drug and alcohol, housing, health, mental health, and other mainstream services, through supported referrals or other appropriate means.
- Develop networks and work collaboratively with support services, government agencies, and other appropriate service providers in order to provide clients with seamless and ‘joined up’ services.
- Assist with program assessments, goal planning, and appropriate referrals of clients to facilitate client movement into stable housing or other supportive living environments.
What you will bring:
- Post-secondary degree, Bachelor of Social Work required along with registration or eligibility for registration with OSCWSSW. An equivalent in the Social Services, Human Services, or related post-secondary degree will be considered along with experience.
- 3-5 years of experience demonstrating the ability to establish effective working relationships through mobile or place-based outreach with people from a wide range of socioeconomic, cultural, and linguistically diverse backgrounds.
- 3-5 years experience in effectively working with adults living with complex needs and vulnerabilities, and homelessness.
- Must meet Hamilton Police eligibility criteria including passing police records, criminal records, judicial matters, and vulnerable sector checks.
- Demonstrated experience in forming strong and productive professional relationships with other agencies in the community
- Strong administrative and planning skills.
- Experience providing client-centered services within a multi-disciplinary team of providers.
- Excellent communication and documentation skills in English are required; fluency in a second language relevant to the community is an asset.
- Ability to demonstrate patience, compassion, non-judgement, and flexibility.
- Non-violent crisis intervention; first aid/CPR; and Food Safe Handler training certification are required.
- Must be able to meet the travel requirements of the position.
