Job Description
Role Purpose
To provide expertise and support in the area of Health management and Health Promotion through the development and delivery of health and clinical services to service users in the Orchard clinic.

  • Support delivery of the organisational strategy and nursing priorities, to ensure high professional standards of nursing practice and patient care are achieved, and a culture of continuous quality improvement is promoted.
  • Lead in the delivery of community liaison nursing services.
  • Lead and contribute to specific nursing related objectives to secure achievement of key performance standards and targets.
  • Ensure services provided are delivered through evidence based and safe care systems.
  • To assume responsibility for his/her own professional development and safe work practice in line with NMBI requirements and be self-directed to manage the role and its development.
  • To maintain accountability for ongoing self-development activities related to service provision, education, evidence-based care, change management and quality improvement.
  • The maintain knowledge and education in regard to evidence-based care in ID settings, health and HSE strategy and national guidelines.
The Person
  • Be a registered RNID or RGN and registered in a Division of the register of Nurses and Midwives in which the application is being made or in exceptional circumstances, which will be assessed on a case by case basis be registered in another Division of the register of Nurses and Midwives.
  • Possess a 3rd level qualification at level 8 (Honours) degree or higher.
  • A minimum of five years post registration (of which two must be in the specialist area).
  • Demonstrate evidence of continuing professional development.
  • A formal post registration qualification in the specialist area of Practice Nursing/Health promotion/Chronic disease management or a related area at Higher Diploma level or equivalent (level 9) on the NQAI framework would be an advantage.

    Relationship
    Job Purpose To manage and support the delivery of optimum healthcare services to
    service users accessing the Orchard clinical services
    Scope of the Role To provide expertise and support in the area of Health management and
    Health Promotion through the development and delivery of health and
    clinical services to service users in the Orchard clinic.
    Key

    Clinical
    • Support and supervise the referrals and triage of Residents to
    the GP service through the administrative role.
    • Support and supervise the provision of GP and nurse led
    appointments for Service users.
    • Lead the daily clinical briefing handover process with the
    medical and nursing team.
    • Lead in the management and delivery of community nursing in
    reach services.
    • Develop, lead and deliver Nursing clinics for Residents and their
    support staff in the areas prioritised by the Director of Nursing
    and GP in delivery of safe care, education, health monitoring and
    promotion of self-management.
    • To ensure the development and delivery of services required
    under the Health Act 2007 in regards to Health, such as AMR,
    audit and screening.
    • Develop, lead and manage database and records in relation to
    Orchard service clinics and records, such as screening,
    medication, etc.
    • Support the GP’s in Kardex/GMS/Protocol management.
    • Liaise and report on the status of hospitalised residents and
    support the communication and safe discharge process.
    • Provide reports and KPI’s where requested by the Director of
    Nursing/ADON.
    • Provide monthly reports in regard to the KPI’s of the Orchard
    service.

    Policy Development & Risk Management
    • Contribute where appropriate to the development and review of
    policies and procedures, which comply with agreed best practice,
    evidence-based care and organisational and national strategies.
    • To identify and develop nursing /care policies, procedures,
    guidelines required by the organisation and relating to the
    outputs of the orchard services in line with evidence based
    care. Actively develop, implement and provide guidance on
    such policies.
    • To adhere to Stewarts Care policies and procedures.
    • Support the delivery of health promotion and education within
    the multidisciplinary process.
    • Develop and maintain the safety statement and risk
    assessments associated with the Orchard service.
    • Record and report on adverse incidents and risks in the
    Orchard clinic.
    • Ensure safe custody, monitoring and management of
    medication in the GP surgery.
    • Ensure the management of clinical supplies through the
    Orchard in line with value for money ethos (Economy,
    efficiency, effectiveness), evidence-based practice and
    structured services.

    Management
    • To act as a professional role model in the development and
    delivery of evidence-based care.
    • To act as line manager to the community liaison nursing team.
    • To act as line manager to the Nursing administrative staff during
    periods for which they are assigned to the Orchard.
    • Provide continuation of service during periods of planned leave.
    • To exercise leadership to ensure that services are provided in
    line with current national policy & guidance for people with
    intellectual disabilities, which promote the rights and inclusion
    of this population.
    • Develop and maintain open communication with professional
    and operational managers, liaise closely with the DON/ADON,
    GP, Specialist nursing team, persons in charge and other
    members of the MDT.
    • Attend and professionally contribute to MDT, clinical and
    specialist nursing meetings.
    • To demonstrate the ability to overcome challenges, propose
    solutions and implement decisions to ensure opportunities are
    realised for people with an intellectual disability.
    • To conduct and engage in audits.
    • To undertake quality initiatives, to participate and lead on
    projects as assigned.
    • To demonstrate measurable outcomes as a result of the service
    improvement, aligned with agreed key performance indicators.
    • To contribute to the development of nursing research
    • To support junior colleagues and mentor nursing students.



    Educational
    • Develop and deliver specific and relevant nursing programmes
    for all disciplines and grades of staff/management and service
    users within the service as assigned by the Director of Nursing.
    • Support service users and staff in the development and review
    of person centred healthcare plans and monitor the
    implementation and effectiveness of same for pharmacological
    and non-pharmacological interventions and therapies.
    • Support service users and non-nursing staff to identify barriers
    and challenges in attending health services and develop
    reasonable accommodations to maximise opportunity.
    • Willingness to undertake training as required by the
    organisation.
    • The Director of Nursing (or their appointee) may allocate the
    postholder special projects to meet the needs of the
    organisation, as appropriate.
    • To be an active member of the Stewarts Nursing research group
    and undertake and promote research-based projects in nursing
    throughout the organisation.
    • Ensure practices on health promotion and health care planning
    are evidence based and meet the specific challenges for persons
    with an ID (Ageing, Reasonable accommodations etc.)
    • Attend and where appropriate make presentations to the
    Nurses forum.

    Qualifications &
    Experience Candidates must meet the following criteria relevant to the role:
    • Be a registered RNID or RGN and registered in a Division of the

    Terms and Conditions of Employment
    Clinical Nurse Manager II – Orchard Services

    Remuneration Remuneration is in accordance with the Department of Health (DOH)
    consolidated pay scales.

    Salary and qualifications are in accordance with Department of Health
    consolidated pay scales. CNMII, scale €56,353 to €66,622 per annum for
    Nursing candidates. Social Care Manager II €56,172 to €64,276 per annum
    for Social Care Candidates.

    Salaries and increments are paid in accordance with DOH scales.

    Annual leave 25 - 28 Days depending on experience

    Working week The hours allocated to this post are 37.5 hours per week, exclusive of
    lunch breaks.
    The allocation of these hours will be at the discretion of the Director of
    Nursing or a person nominated by them and in accordance with the
    needs of the service.

    Probation All employees will be subject to a probationary period of 6 months, with
    an extension if required of 9 months. This probationary period applies to
    all employees irrespective of their role or type of role.

    Garda Vetting Stewarts Care will carry out Garda vetting on all new employees. An
    employee will not take up employment with the organisation until the
    Garda Vetting process has been completed and the organisation is
    satisfied that such an appointment does not pose a risk to the service
    users and employees.
