You will be working as part of our CREST Nursing service as a Case Manager to provide personalised community-based assessments to newly discharged clients with a restorative approach. This involves identifying and resolving issues as and when they arise to maximise rehabilitation potential. This short term service aims to return clients back to their baseline level of function and if that is not possible, to plan for long term support.
This role is about working with our already established team of Nurses, Coordinators, Support Workers and other Allied Health within the community setting. As a Registered Nurse you will provide a broad range of care to clients in their own home such as:
- Passion to deliver exceptional services to people with health or injury related needs in their own homes.
- Demonstrated experience completing assessments
- Ability to develop and review goal focused service plans based on individual client needs
- Excellent customer service
- Ability to work autonomously and as part of a team
- Monday to Friday 8am to 430pm
- Oncall cover once every 5 weeks
- Passion for making a difference and improving the quality of life for people living in their own homes
- Strong communication and organisational skills
- At least 1 years' experience as a Registered Nurse and have a current registration
- Restricted or Full Driver's License - must be able to drive around by yourself!
