An RN's ability to achieve more is enhanced by The Fono's integrated model of care “Fono Care”. The Primary Health outreach RNs is part of a general practice team with not only GPs, HCAs, and Social Workers, but also mental health clinical specialists, Health Coaches, Public Health services, Dental services, Whanau Ora, and a wide range of social services lifting the economic and wellbeing of Pacific families.

- Primary health and/or community care experience
- Capable in chronic care management

The Fono is New Zealand's largest Pacific integrated NGO. We offer an expanding range of high-quality primary health, mental health, dental services, public health, whanau ora, and social services (including a foodbank). The Fono also has the Oceania Career Academy as a service providing, as a NZQA Category One tertiary institution, training, and transitions to employment.
