Job Role: Mountbatten Carer - Bank
Department: Care at Home
Reports to: Care at Home Team Leader
Accountable to: Director of Nursing
Hours: Various hours available, to include weekends, bank holidays and evenings/nights
Salary: Band 2: £11.79 per Hour
Overview of Role:
As a Mountbatten Carer, you will work in accordance with individual care plans written by a named nurse to provide care and support to patients with life limiting illness, their families and carers. You will work flexibly across all care services provided by Mountbatten which will involve working in patients’ own homes or other places of care.
***Full clean driving licence is an essential requirement for this role***
Key Tasks:
· Providing respite care for families/carers as directed by the Senior Nurse in accordance with the patient/family/carer’s needs and care plan.
Professional Responsibilities
· Act at all times in a manner, which illustrates respect for privacy, dignity and confidentiality
· Demonstrate professional behaviour and attitude at all times, with excellent customer care skills and ensure actions support equality and diversity.
· Maintain a level of awareness of palliative care patients and seek advice from the other Healthcare professionals as necessary.
· To participate in induction programmes for new members of staff.
· To understand the systems for accessing the local NHS policies and procedures, for example, Infection Control Policy, Drug Policy and Lone Worker Policy.
· To promote a cost effective and resourceful service.
· Adhere to the Mountbatten’s Uniform and Non Uniform Dress Code Policies.
General

- To deliver direct care, treatment and emotional support for patients as required within set protocols/policies in accordance with the patients individual care plan.
- To contribute to the assessment, implementation and monitoring of patients and carers, identifying physical and/or psychological problems and liaising with the appropriate health and social care team members.
- Work in collaboration with colleagues to ensure the delivery of high quality, safe and compassionate care and support.
- To promote and maintain effective liaison with care team and other health and social agencies involved in the patients care.
- To promote Mountbatten services through good communication with health care providers and the general public of the Isle of Wight.
- Ensure that confidentiality of information is maintained at all times.
· Providing respite care for families/carers as directed by the Senior Nurse in accordance with the patient/family/carer’s needs and care plan.

- To maintain accurate and effective patient records and documentation adhering to national standards, Isle of Wight NHS Trust and Mountbatten policies and procedures.

- Participates in the Service Review, contributing experience to identify service development needs and determine priorities for improving services.
- Promotes patient and carer involvement within all care/practice.
- Provides mentorship for new members of the Mountbatten Bank as required.
· Maintain a level of awareness of palliative care patients and seek advice from the other Healthcare professionals as necessary.
- Report to your Team Leader any infections that they develop which may be transmissible to patients or colleagues.
- Report to your Team Leader any areas where Infection Prevention and Control standards are not being met and challenge any areas where these standards are not met.
- To provide appropriate advice and education to patients, carers and health and social care colleagues, contributing to the learning and development of others as required.
- Has completed or will complete all Mountbatten Bluestream training modules required.
- Undertakes other education as required for the role.
- Takes personal responsibility for continued professional development through clinical supervision, yearly appraisal, relevant training and actively engaging with learning and development opportunities.
- To participate in relevant teaching sessions as required.
- To maintain up to date knowledge of nursing practice attending development training sessions relevant to the post.
