Summary:
The job holder is responsible for co-ordinating telecare across LB Hillingdon; facilitate the change in working practices of health and social care professionals. To work in close partnership with health and social care services and Tele Care Line installation & monitoring teams in order to establish systems, procedures and protocols to ensure the efficient provision of Tele Care Line services. Provide training & development support to social care professional across the customer pathway to support effective assessment for the effective application of telecare solutions included acting as subject matter expert on complex cases
