Description

Are you a new grad or experienced post-acute care nurse practitioner (NP) or physician assistant (PA) who aims to make a difference? Team Health needs someone like you! We are seeking an NP or PA to work in a skilled nursing facility rounding 2-3 days a week. This opportunity includes the flexibility to set your own schedule. We have no start/stop times so you can round in the morning or in the afternoon and still have great work/life balance.

Key Responsibilities:

Perform primary care in a skilled nursing facility setting for short term and long-term patients
Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
Order and interpret lab results
Prescribe and monitor medications
Consult with physicians and coordinate patient care with other healthcare providers
Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
Promote healthy lifestyle changes

Expected compensation is estimated salary range of $64,000 to $81,000 annually with no cap on productivity income potential.
