Responsibilities:

We are seeking dynamic full time Discharge Planner to coordinate hospital care and provide assessment of patient needs through positive interactions to ensure all discharge forms and prevention plans are completed prior to discharge.

Qualifications: Education:
Associate’s Degree in Social Work, Psychology, Professional Counseling or related degree


Experience:
Minimum of 2 years’ experience in mental health

Additional Requirements:
Excellent written and oral communication skills, time management, crisis intervention, and group and individual psychotherapy required. Arizona Dept. of public safety level one fingerprint clearance card. Be at least 21 year of age.
