Referral Nurse Navigator (RNN)

LOCATION: FULLY REMOTE WITHIN CONTIGUOUS UNITED STATES

SUMMARY: The RNN (Referral Nurse Navigator) is the primary liaison between Arista MD clients and our network of specialists, routing referrals, requests, and consultations, ensuring all of the above are routed accurately and appropriately. This person will work within several electronic health record (EHR) systems while leveraging a suite of other technology platforms to accomplish daily tasks.

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Submits accurate e Consults on behalf of primary care providers.
  • Ensures all assigned sites are continually reviewed for e Consult referrals.
  • Follows up on all saved and pending e Consults to be completed and notifies provider of pending items needed for e Consult submission.
  • Ensures accuracy of grammar and structure of e Consults.
  • Routes referrals once a disposition has been made by specialist.
  • Triages and processes submittable vs. non submittable referrals in clinic referral inboxes.
  • Notifies primary care providers with any responses from the Arista MD specialist.
  • Supports RNN team by assisting in busy clinic sites when available or needed.
  • Covers assigned clinic's e Consult referral in baskets, attaches appropriate documents and submits e Consult to specialist, and communicates the recommendations back to the referring provider.
  • Monitors overdue e Consults and communicates information to RNN Manager.
  • Responsible for logging all e Consults which are unable to be submitted.
  • Assists other Arista MD teams in input regarding clinic workflow and Arista MD app efficacy.
  • Quarterly, reviews e Consults per specialty to identify quality improvement areas and complete spreadsheets, as applicable.
  • Reviews completed e Consults to submit for review for QA meeting.
QUALIFICATIONS I COMPETENCIES I SUCCESS FACTORS:
  • Qualifications:
    • Bachelor’s degree preferred
    • Current, unrestricted RN license required
    • 2+ years’ experience in the nursing field
    • Familiarity with e Consults, referral management, and utilization management
  • Competencies:
    • Proficient with any of the major EHR platforms
    • Proficient in Google Suite and Microsoft Suite
    • Comprehensive understanding of various medical specialties and their consultation needs
