Case Co-ordinator - Youth Homelessness - New England

The role of Case Coordinator, HYAP (Homeless Youth Assistance Program) in the New England area is to provide support to the Specialist Homelessness Services and youth work sector in working to prevent and reduce youth homelessness.
The Case Coordinator will also provide intensive therapeutic support for children aged 12-15 and their families to prevent children from becoming homeless or entering the Out of Home Care System. The Case Coordinator will work to support young people’s living situations, improve levels of engagement with family, education and their local community. The Case Coordinator will provide counselling, mediation, group work and practical support to whole families to help break the cycle of homelessness.

- Tertiary qualifications in Social Work or relevant field and experience in the diverse service network of community services
- Demonstrated ability to work in a strength-based way to build and foster connections and support between young people and their families.
- Knowledge, experience and highly developed skills in contemporary early interventions for young people and families that work towards preventing youth homelessness (assessments, family mediation, counselling and conflict resolution techniques).
- Knowledge and understanding of child and adolescent development and the impact of complex trauma on children and families.
- Ability to engage complex families and build relationships with them based on honesty, transparency, professionalism and compassion.
- Working knowledge and understanding of the Going Home Staying Home (GHSH) reform for the Specialist Homelessness Services (SHS) Program, and the child protection framework within NSW.
- Effective communication skills, computer literacy and experience in effective record keeping (including clinical documentation, case notes, and case plans).
- Demonstrated ability to work autonomously and communicate effectively with other services, teams and professionals to accomplish program goals.
