We currently have an exciting opportunity to join the Access and Flow team as a Discharge Liaison Coordinator, where you will be responsible for:
- Working collaboratively with WCH staff to advise and coordinate potential and pending transfers. While acting as a central contact person within Women’s and Children’s Health Network for country clients, families and health professionals.
- Consulting, liaising and collaborating with multi-disciplinary staff and community services to provide a seamless transition for the patient’s with complex high health needs.
- Consulting with key stakeholders will implement and coordinate a comprehensive statewide plan to ensure effective service delivery and efficient management of children and adolescents with complex high health needs.
- Developing sound systems and processes, reviewing and developing policies and procedures, pathways and projects that are established and to enable efficient and effective discharge planning across WCHN.
About You
To be successful in this role, you will have:
- Registered or eligible for registration as a Nurse/Midwife with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
- Registered Nurse and or Midwife with at least 3 years post registration experience.
- Demonstrated competence in the relevant area of nursing and or midwifery practice in accordance with the relevant standards.
- Effective leadership skills including highly developed skills in communication, problem solving, conflict resolution and negotiation skills.
- Experience in discharge planning and assisting patients in the transition to home
- Ability to work effectively within a multidisciplinary team.
