About the Role
This position exists to provide ongoing Nursing Assessment, monitoring, review and support of individual care needs. The incumbent will support individuals receiving support through the Commonwealth’s Home Care Program (HCP) and Short Term Restorative Care (STRC). Emphasis is placed on assisting older Australian’s to continue living independently in their own homes for as long as possible. Home Care Packages (HCP) are provided to Australians who have low to high complex care needs and assessed as HCP eligible by an Aged Care Assessment Team (ACAT). STRC is a Commonwealth funded program that provides a range of care and services for up to 8 weeks to help prevent or reduce difficulties older people are having with completing everyday tasks. Service models employ a Person Centred and Consumer Directed Care (CDC) approach. This approach is most suited to developing goal orientated individual plans supporting a care continuum. HCP’s focus on assisting older Australians address any current health and well-being issues and focus on the principles of enablement and Active Ageing. The incumbent will develop and implement a range of individualised planned supports and/or activities to meet the aims, objectives and timelines within the HCP and STRC programs
What we need from you:
- Current registration as a Registered Nurse with the Nursing & Midwifery Board of Australia
- Experience in clinical care together with undertaking assessments and an understanding of Case Management
- Demonstrated experience in working with people at all levels in a medium to large organisation with multiple sites
- Experience in relevant health/social sciences.
- Previous experience in a community setting.
- Understanding and commitment to implementing Consumer Directed Care and Person Centred approaches.
- Knowledge of local community resources/professional services which support individuals.
- Demonstrated ability to work effectively with a culturally diverse group.
- Demonstrated ability to work autonomously with a high degree of initiative.
- Excellent communication skills, both written and verbal.
- Developed skills in the use of computer software – Word, Excel, Internet and email.
- A current valid Australian Drivers Licence and reliable, insured vehicle.
