Senior Health Clinician – Diabetes Educator

About the role
As the Diabetes Educator you will have the opportunity to work within a dynamic team to provide virtual care to patients with Type 2 Diabetes in a Health District program, supporting patients and their families/carers to broaden their understanding and management of their chronic disease.

Work in a District position to develop partnerships across the health system and with community. This position will participate in innovative models of care and support the local workforce to enhance their knowledge of Type 2 Diabetes.
