Intake Coordinator

Job Summary
Reporting to the Director of Care (DOC), the Intake Coordinator oversees the intake process at Oak Ridges Hospice (ORH) by working collaboratively with the DOC, Medical Director, Clinical Leadership, and Care Team to deliver compassionate care to our current and prospective palliative residents and their loved ones.

The Intake Coordinator is responsible for the processing of all residential hospice referrals to ORH including telephone reach outs, community and hospital initial assessments and reassessments. The Intake Coordinator will also be responsible for maintenance of the deferral list and bed queue in collaboration with the Clinical Leadership Team, DOC, and Medical Director.
Duties
· Fosters a culture of teamwork, compassion and respect that creates effective collaboration between the employees of ORH, to promote a client-centered approach for decision-making and quality hospice care.

· Liaises with prospective residents and their loved ones to ensure strong communication and support is available to our clients from first contact onwards.
· Provides intake and referral information to prospective residents and caregivers calling from the community.
· Makes community visits to assess appropriate prospective residents and caregivers referred to ORH.
· Determines suitability and engages in appropriate follow-up in a timely manner.
· Acts as a resident advocate where needed and desired by the resident and family.
· Participates in resident care conferences with family, team members, and other agencies.
· Meets with prospective residents and caregivers on a regular basis, ensuring continuity of care prior to ORH admission.
· Promotes ORH in the community, providing education and awareness within the medical community and palliative and community support sectors;
· Collaborates with other organizations in the delivery of hospice and palliative care services.
· Serves as a member of the Clinical Leadership Team providing support and promoting excellence in our delivery of clinical services.
· Provides coverage within the clinical services team in place of the Director of Care and/or Clinical Leadership positions during periods of vacation or absence.
Qualifications:
· Registered Nurse in good standing with the College of Nurses Ontario with the requisite practice insurance.
· BSc N or RN diploma with demonstrated commitment to continuous learning.
· Certification in Hospice Palliative Care Nursing (C)anada (CHPCN(C)) preferred.
· LEAP Certificate (Learning Essential Approaches to Palliative Care), Fundamentals of Hospice Palliative Care, Enhanced and Comprehensive Advanced Palliative Care Education (CAPCE) Certification or willingness to complete within 1 year of hire*
· Demonstrated commitment to on-going education and professional growth to maintain current knowledge in the changing face of hospice palliative care.
· Minimum 3 years of clinical experience in palliative care, with hospice experience ideal.
· Demonstrated experience in client assessment and patient advocacy.
· Evidence of a demonstrated a client-centred approach to care and ability to work with families.
· Demonstrated expertise with pain and symptom assessment and management.
· Physical ability and emotional resilience to provide care to clients.
· Knowledge of interdisciplinary care processes, commitment to team-work and ability to collaborate effectively.
· Experience working with a wide variety of community service providers.
· Demonstrated understanding of ethics, especially as related to the provision of hospice palliative care.
· Ability to foster a high-functioning team approach and a commitment to mentorship of clinical staff.
· Participates in mandatory education/training in-services for skill development, self-care, teamwork, and updates on new and/or relevant information on best practices.
· Strong computer skills (Word, Excel, Outlook, databases, and effective internet research) and a willingness to learn new systems.
· Superior communication skills, both oral and written.
· Strong decision making and problem-solving skills.
· The ability to work flexible hours/travel.
· Provision of Police Record Check and Vulnerable Sector Screenings.
· Valid G-class license and requisite car insurance
Additional Requirements:
· Require travel within the Durham Region; access to reliable transportation needed.
· As a member of the Clinical Leadership Team, support the Director of Care and Clinical Leadership team when needed.
Oak Ridges Hospice recognizes the importance of immunization to protect our residents, team members and visitors from COVID-19. As such, a condition of employment with Oak Ridges Hospice is requiring that all employees and new hires have received all required doses of a COVID-19 vaccine approved by Health Canada.
Job Types: Full-time, Permanent
Pay: $39.80-$44.96 per hour
Expected hours: 37.50 per week
Education:
  • Bachelor's Degree (preferred)
