The Faith Community Health Coordinator position is designed to support the Faith Nursing Program at Franciscan Health. This role collaborates with senior and departmental leaders to set priorities and develop strategies for advancing the Faith Nurse program within the Franciscan Health system and in the larger community.

WHAT YOU CAN EXPECT

Create and steward collaborative interdepartmental and community relationships to strategically leverage partnerships and resources to advance the health of our community.

Participate in meetings, serve on committees, and represent the department and hospital/facility in community outreach efforts as appropriate

Coordinate all aspects of the Faith Nurse program, including educational opportunities, meetings, and resource support. Collaborate with members of the program to offer health-focused programming and events to benefit their places of worship and the larger community.

Support community initiatives to promote collaborative arrangements among Franciscan Health, community partners, and general community members to ensure programming consists of best practice standards for optimal health.

Attend appropriate conferences and meetings as the hospital's representative and shares findings with leadership as necessary.

Coordinate creation of community benefit reports and ensures accurate reporting for the state government, federal government, and local community benefit reporting.

Assist with the Community Health Needs Assessment and Community Health Improvement Plan. Provide support for the community health needs assessments and developing dashboards for implementation strategies.
QUALIFICATIONS
-
Bachelor's Degree in Public Health or Allied Health Profession certificate required
-
Bachelor's Degree in Nursing or Health Care related preferred
-
3 years of working with patients and/or community members in a health/public health setting required
-
1 year of working with community organizations preferred
-
3 years of Community or Faith Nursing preferred
-
RN, LPN, CMA, AAMA, or EMT preferred
-
Valid Driver's License
required
- Local travel within or near Marion County and occasional travel to other Franciscan regions
