Clover LLC is now hiring a SPEECH THERAPIST for Clover Home Health Care a provider of home care and home health care services based in Carmel, Indiana and servicing patients in the greater Indianapolis area. The Speech Therapist, under the supervision of the Director of Nursing, performs patient care as directed by way of a written plan of care and in accordance with state practice guidelines.
You must be highly motivated, provide excellent quality clinical care, be engaging, caring and patient. Requirements:

• Current Speech Therapy License in the state of Indiana.
• Valid drivers license, valid auto insurance, and reliable transportation.
• Current CPR/In-Person Certification
• Valid TB test (past month) or have one placed in our office.
• Tech savvy, able to learn and navigate several platforms. Proficient with Apple products.
• Perform routine duties independent of supervision but consults with others as required.
• Excellent communication and listening skills.

Preferred:

• Proficient in using EMR systems, (AXXESS).
• Minimum of 2 years Home Health experience.
• General knowledge of home health practices, including familiarity with relevant state and local services.
• AXXESS EMR knowledge

Responsibilities:

• Direct patient care (Start of Care, Patient Evaluation, Resumption of Care, Discharges, Transfers, Speech Therapy Visits)
• Design and implement a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.
• Document patient care services by charting in patient and department records.
• Performs therapeutic procedures by administering manual exercises and instructing, encouraging, and assisting patients in performing physical activities, such as non-manual exercises, ambulatory functional activities, and daily living activities.
• Accesses patient health by interviewing patients, performing physical examinations and obtaining, updating and studying therapy histories.
• Provides direction and leadership to staff and acts as a resource to promote speech therapy in the community.
• Oversee's new hire competency check offs.
• Practices Home Health policy and procedures, objectives, quality improvement programs, safety and environmental and infection control standards.
• Maintains a safe and clean working environment by complying with procedures, rules, and regulations, and adhering to infection-control policies and protocols.
• Maintains production objectives by noting current treatment trends and monitoring actual speech therapy services rendered to production objectives.
• Maintains legal and accreditation compliance by adhering to policy positions concerning federal, state, and local regulations and JACHO standards.
• Updates job knowledge by participating in educational opportunities, reading periodicals, maintaining personal networks. Enhances speech therapy reputation by accepting ownership for accomplishing new and different requests and exploring opportunities to add value to job accomplishments.
