Position Summary: Sisters Together Community Health Worker (CHW) will play a key role in promoting health and wellness within the community by focusing on diabetes management, blood pressure control, and physical activity outcomes. The CHW will work closely with individuals and families to provide education, resources, and support to help them achieve healthier lifestyles. This position is ideal for someone passionate about community health and looking to make a positive impact at the grassroots level.
Duties and Responsibilities
Outreach and Education:

• Conduct outreach to community members, providing education on diabetes prevention, blood pressure management, and the importance of physical activity.
• Distribute educational materials and resources related to healthy eating, exercise,and chronic disease management.
• Facilitate group workshops and one-on-one sessions to educate clients about managing their health conditions.

Client Support:

• Assist clients in setting personal health goals and developing action plans to achieve them.
• Provide ongoing support and follow-up to ensure clients are adhering to their health plans.
• Help clients navigate healthcare systems and connect them with appropriateresources, such as medical providers, nutritionists, and fitness programs.

Monitoring and Reporting:

• Track and document client progress in managing their diabetes, blood pressure, and physical activity levels.
• Maintain accurate and up-to-date records of client interactions and outcomes.
• Report on program outcomes to funders and provide feedback to supervisors on potential improvements.

Community Engagement:

• Participate in community events, health fairs, and other outreach activities to promote program services and encourage community involvement.
• Build relationships with community organizations, local businesses, and healthcare providers to enhance program outreach and resource availability.

Advocacy and Referral:

• Advocate for clients' needs within the healthcare system and community services.
• Refer clients to appropriate healthcare services, social services, and community resources as needed.

Qualifications

• Education: Some college coursework in public health, social work, or a related field is

preferred.
• Experience: Previous experience in community outreach, health education, or working
with underserved populations is required
• Physical fitness: Ability to lead some physical activities like walking programs.
Certifications in yoga or other fitness activities a plus.
Skills:

• Strong interpersonal and communication skills, with the ability to connect with diverse populations.
• Basic understanding of chronic conditions like diabetes and hypertension, and the role of physical activity in health.
• Ability to work independently and as part of a team.
• Organizational skills and attention to detail in maintaining records and documentation.
• Familiarity with the community and local resources is highly desirable.

Certifications: Community Health Worker certification is a required

• Willingness to obtain certification is required.
