Health and Wellbeing Co-Ordinator
OFFICE BASE from Convent of Mercy, Kells, Co Meath, A82 C7P3
JOB PURPOSE
The Health and Wellbeing Co-Ordinator will ensure a system is in place for the provision of support and care as required to members of communities, with the dignity of each Sister being respected and upheld at all times.
ESSENTIAL CRITERIA

• A registered General Nurse on the Register of Nurses held by Nursing and Midwifery Board of Ireland.
• Minimum of 4 years’ experience working at Band 6 (or equivalent) in either general medicine, older people’s services or primary care.

OBJECTIVES

• To conduct the role of Health and Well-being Coordinator within the traditions, philosophy and spirit of the Congregation of the Sisters of Mercy and in adherence to the charism of Catherine Mc Auley.
• Ongoing evaluation of objectives, strategies and action plans and to implement those action plans.
• Ongoing review, development and evaluation of Policies and Procedures for the conduct of the role and the delivery of its services.
• To establish personal objectives for service within the role; to evaluate personal performance and to undertake professional development programs in consultation with and approved by the PLT.
• To advise the Provincial Leadership Team if the living conditions of an individual are suitable to their needs or acquire to be addressed.
• To liaise with the Healthcare Team (HCT), Cluster Leaders and Managers.
